Dementia: Options for Help In and Out of the Home


For patients and care partners of persons with dementia, decisions about long-term care and support can be challenging and may feel unexpected.  As dementia progresses, physical, social, and emotional needs will change over time.

This support tool was designed as a first step overview to anticipate and review options for when it is time for increased support at home, programs in the community, and/or assisted living or memory care.  There are no right or wrong choices, each family and situation is different with many unique factors. We encourage you to review this document with a member of your healthcare team who can help guide you further to specific next steps and considerations.
